Q&A 25 Modifier Needed Well Child Visit when billing with vaccines?

I cannot seem to get a clear answer on this:

Is it appropriate and necessary to append a 25 modifier on a well child visit when billing the well child exam with vaccines and vaccine administration? I have seen threads on the AAPC about this that are conflicting. Some people post that you do not need it on the well child visits, while others are stating that you do need the 25 on the E/M.

Please help.
